In the ancient Empire of Mahishmati in south India, Sivagami, the queen mother (Ramya Krishnan), emerges injured from a cave adjoining a big waterfall, carrying a baby. She kills the soldiers pursuing her and sacrifices herself to save the baby by holding the baby up over the surface of the water while she herself drowns. Local tribal communities save the baby and the tribal queen, Sanga (Rohini) adopts him. She also gives him the name 'Shivudu'. Sanga also orders her tribesmen to lock the passage along the waterfalls, so that her son would never leave her.
Shivudu (Prabhas) grows up to be a strong young man aspiring to ascend the waterfall and makes many unsuccessful attempts at it. One day, he finds a wooden mask of a girl which falls from the waterfall. Driven to find out the identity of the girl, he climbs the mountain again and succeeds this time.
On top of the waterfall, Shivudu discovers that the mask belongs to Avanthika (Tamannaah), a rebellious warrior whose group has indulged in a guerilla warfare against the emperor Bhallala Deva (Rana Daggubati) of the Mahishmati Kingdom. The group intends to rescue their former queen Devasena (Anushka Shetty), who has been chained open in the Palace for the past 25 years. Avanthika is given the opportunity to rescue the queen, in the upcoming festival.
Shivudu approaches Avanthika while she is on her journey to Mahishmathi. Although she is stubborn at first, Avanthika falls in love with Shivudu after she finds out that he has climbed the waterfall for her. Shiva pledges to help her in her mission and sneaks into Mahishmathi to rescue Devasena. At the same time, the emperor has plundered all the gold of the civilians and has had it made into a single large statue of him. The statue is 100 feet tall, and is to be established in the center of the town in the fest. In the time of the establishment, there occurs a mishap and the statue is about to fall down and crush 100 workers to death, when Shivudu (in disguise) , comes and helps them to safely erect the statue. But one slave sees Shivudu's face, and starts chanting " Bahubali", which then echoes through all people, insulting emperor Bhallala's ego. In the evening, Ballala investigates this amongst his palace gaurds, while Shivudu reveals himself, and successfully evades the gaurds to meet Devasena. Shivudu rescues her and flees along with her but is chased down by the king's royal slave Kattappa (Sathyaraj), known for his great warrior abilities. After Shivudu beheads the only son of Bhallala, Prince-Bhadra (Adivi Sesh), Kattappa drops his weapons on realising that Shivudu is Mahendra Baahubali, the son of the late king Amarendra Baahubali (Prabhas) and Devasena.
A flashback reveals Amarendra Bahubali's past. Amarendra's father Vikramadev was chosen as king of Mahishmathi much to displeasure of his elder brother Bijjaladev (Nassar) who was infuriated because he felt that he was being discriminated against due to his disability. Vikramadev died leaving Mahishmathi's throne empty, while his queen died after giving birth to Amarendra. Sivagami, Bijjala's wife, took charge of running the kingdom with Kattappa's assistance until a new king is elected, while also taking responsibility of nurturing infant Amarendra, naming him Baahubali and raising him along with her son Bhallala Deva. They are brought up together, trained in all areas including arts, science, disguise, politics and warfare, but both of them have different approaches towards kingship. Amarendra Baahubali (Prabhas) is liberal and kind to everyone but Bhallala Deva (Rana Daggubati) is ruthless and achieves his goals by any means possible; he even plots to kill Bahubali along with his father many times.
When the Kalakeyas start a war with the Mahishmathi, Sivagami promises that the new king will be the one who beheads the Kalakeya king, she also declares that weapons must be equally divided between them. But Bijjaladev plots for Bhallala's win by providing superior weapons for him and leaving inferior ones for Bahubali. In the battle, when it seems that Mahishmathi will be defeated, Amarendra inspires his soldiers to fight to the death and they decimate the enemy forces. Despite Bhallala Deva being the one to kill the Kalakeya king (Prabhakar), Sivagami announces Amarendra Bahubali as the new emperor because of his nobility and leadership in the battle.
After the flashback, when asked about Amarendra's current whereabouts by Sanga, a tearful Kattappa reveals that he is dead, and the movie ends in a cliffhanger that Kattappa himself is the one responsible for his death.
